	Purpose of form Please use the form below to submit your design selection to our Design Team. Please make sure you have looked through our design guide prior to submitting your request, as it details the design process and should answer any questions you have about the various design elements that encompass your design selection. Please be aware that once sent, we consider this to be your formal selection and we will base your design upon the choices you have made in this form. If you change your mind AFTER you have submitted this form, please let us know immediately. Once designed, any major design changes you request that are contrary to your selection may result in additional charges. Please be aware that selecting rotating banner images and cascading navigation may impact the final cost to your organization.
